This young woman had 2 previous breast augmentations performed by another surgeon. At the second procedure, she had ultra high 400 cc silicone implants placed through an incision on the upper half of her areola, and had developed severe capsular contracture on her left breast which was causing her significant pain as well as difficulty fitting comfortably in clothing. She was interested in slightly smaller implants, more symmetry, correcting the contracture and equalizing the position of both breasts relative to each other. I performed breast revision surgery, during which I removed her old implants, excised her old hard capsules, reconstructed her implant pockets with silk scaffold, and placed new, smaller textured round implants (350 cc) all through new incisions hidden under her breasts, and also revised her old widened areolar scars. This created a very nice, balanced, and softer breast appearance, as well as significantly improved the pain she was experiencing from the capsule, as seen in her post-op photos taken about 5 weeks after her procedure.
