This woman in her 50s had had breast augmentation originally 25 years ago with implants placed behind her muscle, followed by removal of her implants years later. She was now ready to have implants placed again. On examination, I noticed that her muscle had retracted significantly from her previous surgery. We placed new cohesive gel silicone implants and to control the coverage of the implant with her muscle as well as to offer more support to her implants, we also used silk scaffold as an “internal bra”. This was a critical part of her procedure that allowed her to get a very nice, natural, and durable result as seen in her post-op photos.
