How Fat Transfer Works
In simple terms, fat transfer (also called fat grafting) means moving fat from an area where you have a little extra to an area that needs more volume or better contour. Surgeons typically remove the fat with gentle liposuction, clean and concentrate it to keep the healthiest fat cells and then inject it back into the body in many tiny droplets so those cells have the best chance of picking up a new blood supply and surviving.
Over the last several decades, plastic surgeons have used this technique not just for cosmetic reasons, but also to repair contour problems after injury or surgery. Because the material is your own tissue, it integrates with your body and tends to look and feel very natural.
Common Donor Areas: Where the Fat Comes From
When planning a fat transfer, we first identify suitable donor sites. Typically, the abdomen, flanks (love handles), thighs, lower back, or hips. These are the same areas commonly addressed with liposuction, making them ideal for harvesting excess fat for grafting while simultaneously contouring the body.
Choosing donor sites this way means we can both collect healthy fat and subtly improve your body contour at the same time. Patients with at least a moderate amount of extra fat give surgeons more options for where to harvest and how much volume we can safely transfer. For smaller procedures like facial fat transfer, we may only need a modest amount from one area; for breast or buttock work, we need more generous donor sites.
Where Fat Can Be Transferred
Face and Mid Face
Fat transfer is especially useful in the face, where age related volume loss can make you look tired or drawn even when you feel great. Facial fat grafting refills hollow or wrinkled areas, softens creases, and can add volume to the mid-face to restore a youthful appearance.
Unlike temporary fillers, transferred fat integrates into your own tissue, providing long-lasting structure with a completely natural look and feel. And the bonus? That stubborn fat you’ve always wanted gone gets repurposed exactly where you want it most.
Breasts
Fat transfer to the breasts is an excellent option for women seeking a subtle enhancement rather than a dramatic size change. It can create the upper pole fullness many women desire, improve the transition between the chest and breast, or refine shape after previous surgery and we can place the fat precisely where it’s needed most. Best of all, there’s no implant to worry about. It’s entirely your own tissue, giving you a natural look and feel that nothing artificial can replicate.
Recovery looks a bit different for each person, but most patients plan for one full week of downtime of no working and no driving. Around the one-week mark, most people are able to return to work, provided their job fits within our six-week physical activity restrictions.
In practice, fat transfer to the breast can:
- Provide a small but noticeable size increase with very natural softness
- Be performed on its own or combined with a breast lift or reduction to restore upper pole fullness for a more complete result
- Be a valuable tool for smoothing implant edges, correcting dents or irregularities in the breast, and is frequently used in breast revision cases to refine and improve prior results
Buttocks and Hips
Fat transfer to the buttocks and hips (often called a Brazilian butt lift, BBL, and Hip Dips) combines liposuction of the waist/flanks, among other donor sites, with targeted fat injection to the buttocks and hip area. Patients seek this type of gluteal enhancement especially when they want to create smoother curves or improve the waist–hip ratio.
When done conservatively, using controlled moderate volume and in the right tissue layer, this approach can:
- Increase upper buttock fullness and projection
- Smooth “hip dips” and create a more continuous curve from waist to thigh
- Maintain an elegant, proportional shape rather than an exaggerated look
Regardless of target site, the goal should always be a smooth, blended transition between treated and untreated areas, so your body looks like it has always been shaped that way.
Recovery and Fat Transfer Results
Recovery looks a bit different for each person, but most patients plan for one full week of downtime, including no working or driving. BBL patients will also need to refrain from sitting on their bottom for one week. Around the one-week mark, most people are able to return to work, provided their job fits within our six-week physical activity restrictions. Swelling and bruising are most noticeable in those first couple of weeks and gradually improve from there. Over the following months, the treated areas continue to settle as swelling resolves and any non-viable fat is naturally absorbed by the body. Final results can take up to one year to fully reveal themselves, so patience is key.
Once the transferred fat has stabilized, any changes are typically due to normal aging or weight fluctuations rather than the graft itself, which is why we encourage patients to maintain a stable, healthy weight and lifestyle to protect their results.
Benefits Compared with Fillers or Implants
For many patients, fat transfer sits in a sweet spot between traditional fillers and implants:
- It uses your own tissue, so true allergic reactions are essentially not an issue.
- Once the initial healing phase is over, the improvement can last much longer than most office based fillers, which are often measured in months or a couple of years.
- It is especially useful when we need to restore global or structural volume, like an entire mid-face or the buttocks, whereas fillers excel at tiny, focused tweaks.
The trade off is that fat transfer is surgery, with downtime and some unpredictability in how much volume ultimately survives, whereas fillers are quick and reversible but need repeated maintenance.
Who Is a Good Candidate?
The ideal fat transfer candidate is someone in good overall health, near a sustainable goal weight, who does not smoke (or is willing to stop), and who has enough extra fat to harvest from donor areas. Just as important, the candidate understands the procedure, its risks, and its realistic outcomes. Generally speaking, we look for candidates who:
- Are medically stable without serious uncontrolled conditions that increase surgical risk
- Have some extra fat in common donor zones like the abdomen, flanks, thighs, or hips
- Have skin quality that can support improved contour
- Are looking for subtle to moderate changes, not extreme enlargement
