At your initial consultation, you will review the pricing of your procedure with our Patient Care Coordinator, who will provide you with a quote in writing. Payment for your procedure is required at least three weeks before your procedure. We accept Mastercard, Visa, Discover, Cash, or Check. In addition, many patients today choose to finance their surgical procedures, and we offer financing through CareCredit. CareCredit offers a full range of payment plans, including 6-month No Interest payment plans featuring monthly payments as low as 3% of the balance and let you pay within the specified time without incurring interest charges. For more information, go to www.carecredit.com or call our office at 206.467.1101.
A $1000.00 NON-REFUNDABLE scheduling deposit is required at the time of scheduling a surgery date.
Your $1000.00 deposit will be applied towards your surgery balance.
Full payment for surgery is due at the Pre-Operative appointment, which is scheduled to take place at least three weeks prior to surgery, and once paid, is non-refundable. If fees are not rendered at that time, surgery may be cancelled. The Pre-Operative appointment is the definitive and final consultation prior to surgery. Mastercard, Visa and Discover cards are accepted and have a 3% processing fee.
The only exceptions to our refund policy are life-threatening emergencies or medical illnesses that prevent the patient from safely proceeding with the intended surgery. Complete documentation of such an event by a specialist will be required to receive a full refund.
We require 30-day notice to reschedule your surgery. Any surgery rescheduled outside of 30 days will not be penalized and your $1000.00 surgery deposit will be applied towards your rescheduled surgery.
If lab work or medical clearance is required, it is the financial responsibility of the patient. If not completed, surgery may be canceled without a refund.
While the overwhelming majority of patients are quite happy with their results, on occasion a touch-up procedure is required. These situations are handled on a case-by-case basis, but in general, should a secondary procedure be deemed necessary by Dr. Salemy, you will be responsible for the operating room and anesthesia fees. The surgeon’s fee may or may not be waived based on the particular situation.
